Care Team Nurse RN/Psychiatric Services RN

Location: West Salem Clinic | 1233 Edgewater St. NW, Salem OR 97304

Job Status: Full-time, Monday-Friday with rotating Saturdays

Sign-on Bonus: $3000 (Internal candidates that are currently employed here at NWHS are not eligible for a sign-on bonus.)

YOUR ROLE:

Your role as a hybrid RN is to provide practice management support to the clinical and psychiatric team with other healthcare professionals to address patients’ wellness, acute illness, chronic disease, psychiatric needs, disability, and end-of-life needs. You will oversee the respective provider’s assigned patient panel by directing and facilitating care to support the patient and incorporating the nursing process: assessment, planning, intervention, and evaluation.

SPECIFIC DUTIES:

  • Participates in the ongoing management of patients with stable chronic diseases, as designated by and in consultation with the Primary Care Provider (PCP). Communicates with patients in person and over the telephone regarding issues concerning management of their chronic disease, coordination of treatment plans/changes, as well as coordination of services. Includes follow-up with the patient when indicated.
  • Manages the triage, intake and monitoring of patients with acute medical conditions. Monitors patients in the absence of their PCP, providing necessary nursing intervention when appropriate.
  • Under the direction of the PCP, provides patient education in areas related to self-care, such as taking medications appropriately, use of nebulizers and inhalers, diabetes management, wound care, and self-injection, as appropriate. Explains treatments and procedures to patients.
  • Participates in the ongoing management of patients with mental health diagnoses, as designated by and in consultation with the Psychiatric Provider. Communicate with patients in person and over the telephone regarding issues concerning the management of their mental health diagnosis, coordination of treatment plans/changes, as well as coordination of services. Includes follow-up with the patient when indicated. Providers care via telephone or in-person appointments for reassurance and to monitor for side effects until future appointments with their prescriber, for example starting a new medication.
  • Demonstrates strong critical thinking skills, able to maintain composure during challenging events, able to de-escalate clients and can problem solve effectively. Is a resource to the support team, proactively responds to clients in crisis in the lobby if a Behaviorist team member is unavailable, deescalating the situation and making a safety plan if needed. Communicates effectively in a professional manner with clients, staff, and providers updating the team of a change in client presentation.
  • Huddles throughout the day with PCP and Care Team Medical Assistants, identifies deficiencies in workflow, and proactively collaborates with the entire care team to problem solve. Performs point-of-care testing such as collecting lipids and A1C lab values including entering NextGen. Performs ECG testing when needed.

QUALIFICATIONS:

  • Current Oregon RN License
  • Has a current Basic Life Support for Healthcare Providers (BLS) training certificate consistent with AHA Guidelines for CPR/ECC, and covers breathing and cardiac emergencies - including CPR, AED, and obstructed airway - for adult, child, and infant patients.
  • Clinic and psychiatric experience preferred.
  • Ability to work well in a group setting.
  • Ability to prioritize competing responsibilities while under pressure.
